since when is “free” $76.39?

Went in for my annual free physical, and insurance covered everything—except a $76.39 vitamin D test. Hospital says it’s a “basic test.” Insurance says it’s “not routine.” Neither bothered to ask if I wanted to pay. If something isn’t covered, the hospital should tell me upfront so I can decide. Instead, I get a surprise bill after the fact? Nope. I told hospital: either waive it or fight it out with my insurance company. Not my problem.
