Anyone else had weird billing issues?

Went in for a routine checkup at my family doctor’s office, but the bill came back as an 'on-campus outpatient hospital' visit. My insurance won’t cover a cent because of that label. Has anyone else run into this? Feels like a total scam. #healthcare #insuranceissues #Health #BodyHealth
